Important information

Form Submission

  1. Click on the “Provider Login” navigation on the menu tab.

  2. Enter the password provided to you to access the page.

  3. Start by filling out your name and email. Your name will be in the dropdown, and you can start typing it out to click on it. If you can’t find it on the list manually enter your name.

  4. Select the days you want to input in the current submission. This is NOT the amount of days your worked this week, it is the number of days you want to provide information about.

  5. Enter information for each day: Location, Date & Hours Worked, Shift Start/End Times, whether you worked overtime, and any comments.

    • Location: It is a dropdown menu. You can search the location to single it out.

    • Date Worked: You can click on the calendar icon to select it or enter it manually.

    • Hours Worked: Input total hours worked that day.

    • Shift Start/End Time: you can click on the clock icon to select the time your shift started and ended or enter it manually.

    • Overtime?: Check yes if you worked overtime, no if you did not.

    • Comments: Please enter any comments that are important for us to know when processing your billing.

  6. Click “Review Submission”.

  7. Please review your submission carefully. Click “Edit” if you need any changes made. Click “Confirm” if you’re happy with the submission.
